The following items need to be uploaded while completing the enrollment form (.pdf, .jpg or .png file format):

Physical Form - This is a record of your child's most recent well check. 

Dental Form - This is a record of your child's most recent dental checkup. 

Child’s birth certificate or hospital record

Child’s Immunization Records

IFSP/IEP, if applicable
